Kidney Wellness Hub - Focus Group Scheduling Survey

Thank you for your interest in joining your kidney community for a 1-hour virtual focus group over Zoom!

This is your chance to:

• Connect with others in the kidney community
• Share your needs as a member of the kidney community
• Tell us what content you would like to see on the Kidney Wellness Hub

This focus group is part of an evaluation of the Kidney Wellness Hub and will be hosted by two staff members from Reichert & Associates, an independent evaluation firm.

Kidney Foundation staff will NOT be present for the discussion and focus group participants will be compensated $25 for their time and experience shared. Please note that your decision to participate is voluntary and will NOT affect your access to the Kidney Wellness Hub or other services offered by the Kidney Foundation.
