About the Survey

This survey aims to learn about the experiences of people living in the South Island/Victoria region who do not have a primary care provider and access to cancer screening services. 

The following questions will ask you about your experiences with accessing care, including what is currently working well and where there are gaps. The information you share will be used to develop a needs assessment report for a Shared Care project. This project is working to develop a cancer screening and surveillance service model to support patients without a primary care provider in the greater Victoria and South Island region. Your participation will provide us with valuable information that will be used to improve the primary health care system in our community.

Who should complete this survey?

To complete this survey, we are looking for people who:
  1. Have accessed or tried to access cancer screening services in the last 2 years;
  2. Did not have access to a primary care provider during that time;
  3. Live in the Victoria/South Island region; AND
  4. Are 30+ years old
 
For the purpose of this survey, cancer screening includes publicly-available cancer screening tests, which include breast, colorectal, lung and cervical cancer screening.
