We want to hear about your experiences with mental health and substance use care in the Outer Southern Gulf Islands (Saturna Island, Mayne Island, Galiano Island, and Pender Island). Your input will help local care providers understand how residents experience finding and accessing mental health and substance use supports. The information will be used to guide the project and improve how information and connections to existing services are shared.
The survey will take approximately 5 minutes to complete. At the end of the survey, you will be invited to enter a draw to win a $50 gift card.
If you are unable to complete the survey on your own, you may ask someone to complete and submit the survey on your behalf.
Your privacy is important. We won’t ask for any personal details. All responses will be combined into a summary that will be shared with the project team at the Rural and Remote Division of Family Practice. The survey is voluntary, and all questions are optional.
