The South Island & Victoria Primary Care Network Community Advisory Groups want to hear from you! Your experiences with family doctors, nurse practitioners, and care teams matter—and your voice can help improve how care is delivered in our community. Primary health care is your day-to-day care from a family doctor, nurse practitioner, or their team. They treat common issues, support your overall health, and connect you to other services when needed—like counselling, specialists, or surgery.
The following 15-minute survey is designed to find out your experiences and how we can make the system better for you. To thank you for your time you will be invited to enter a draw to win a $50 gift card (to Tim Horton’s or Real Canadian Superstore) at the end of the survey.
The information collected in this survey will be provided to the South Island & Victoria Primary Care Network Community Advisory Group and Steering Committee for planning and improvement purposes only. Last year, we were able to use the information we collected to reallocate more resources towards mental health and raise awareness of needs of unattached patients (people who don't have a doctor or nurse practitioner).
All survey responses are voluntary, anonymous and will be combined and reported in a way that does not identify any individual person. If you are unable to complete the survey on your own, you may ask someone to complete and submit the survey on your behalf.
